Dush jee mostly asks abstract questions , questions which can be interpreted in many ways and will have many answers. So you can give your answer based on your understanding of the question.
Now back to the question. I am guessing , she is referring to situations where some doctors in Pakistan will not give the whole news to a patient who has terminal disease , then a patient who has received this kind of news about himself will hide it from the family etc.
I guess in doctors case he knows that the patient will be devastated to know what doctor knows, and then in case of patient not informing the family it is out of love for the family he does not want them to agonize on his misery.
