It seems intelligence failure is quite ripe in this organisation. Mumbai attacks was not first one, many happened before as well like the attack on Parliament etc., It is only after the event that suddenly they wake up and realise that ISI was behind the plot.

Over beareucratization is problem of this organization, the biggest debacle was kargil as they were unable to know that pakistanis are now on hill tops, but not able to disguise as muslim by growing beard seens far more absurd, funds of this organization is never budgted and audited too, ISI is/was smarter than RAW probably, I am damn sure RAW cannot hide Osama in the way ISI did, only amazing thing is being known world over for manual intellgence gathering ISI is unable to stop daily bombing in Pakistan, Mumbai was much lesser than Kargil but it is Indian diplomats who use everything to their advantage, Indian Army would have taken much longer to remove infiltrators from hilltops had it not been diplomacy, they say it to the army openly, we gained more than half of what you lost:D
