This question requires extruding from Islamic sources. If we assume the phones storage capacity to be analogous to a book then your concerns stand true, if we assume the phones storage capacity to be analogous to the mind of the hafiz, such as memory, then we can appoint a different ruling. We don't take books into the toilet but we do take out minds to the toilet.

Generally speaking if the phone is on and in the middle of tilwat then this should be switched off. However, if you want to be extra prudent then it is better to switch off the phone, put the phone away or not take it in the toilet with you in the first place. This is my understanding and not a ruling. If you want a ruling you will need to go to a Mufti.
