Yes it happened to me in Islamabad Airport, but not by the Immigration People, I had to hand in the Passport to recover unaccompanied baggage, someone tried to remove my picture from the passport, it only tore off one corner. the Immigration counter caught that and asked for a second ID because the passport picture was eight years old!
I learned you have to be attentive when it comes to travel documents.
Also No one believes I was born in Hawaii…
