we used to fill out

• a form duly attested by 17 grade officer;
  send it to admin
  along with
• 3 passport size picture,
• birth certificate
• 3 references from GS
• NOC from local police station
• a copy of NIC and Driving licence
• and most important (jawabi lifafa)
