yes i have done it just once…
i usually dont forget it because my uncle was my driving instructor and he made sure i took the key out of the ignition before unlocking ma seatbelt so it has become a habbit.
another habbit i have is to lock the car when i get out… so once i wanted to get somthing from the trunk and as i opened the trunk and got out of the car, i unconciencely locked the doors 
that time i dint have a spare key, which i usually kept in ma wallet just in case 
with my new civic, the only way to lock it is to lock the driver’s door by using the key and all the other doors lock automatically so there is no chance of u forgetting it inside
