Depends on how long you want to drive in Pakistan. If its just a few weeks of vacation, then, by law, you can drive on a valid drivers' license from US (or any other country). You don't necessarily need an International Drivers' License. Chances are, as long as you don't get stopped for any traffic violation, this will be a moot issue.
Although, if you do get caught, then the policeman may not know all the legal issues, and may insist to see a Pakistan or an International Drivers' License. So its always helpful to get an International Driver License available. If you don't have one, you can still talk your way out of it by explaining that you have a valid drivers' license and, by law, you can drive on it for upto 3 weeks in Pakistan.
In most cases, if they want to give you a ticket, they will just confiscate the registration papers of the car, and once you pay the fine at the court, you can get the papers back. So having an international drivers license or anything like that (or lack thereof) is not likely to hamper their ability to give you a ticket, or your ability to drive.
