I think a cologne or an article of clothing ... there are tons of british brands that we don't get in north america.

If you are on a budget. We are going to be having thanksgiving sales soon here in USA. You can probably order something online & get it delivered to him & just mail a birthday card separate. Most websites offer free shipping on any purchase above 50 dollars & provide gift wrapping for an additional 2-10 dollar charge. I am sure with sales & some good shopping around on the websites you'll find something for him.
Also some websites also give you an option of sending a gift card.
& since you are confused about what to buy. If you are comfortable enough why not ask him what he likes. Let him know your price range & that how you would really love to get him something he wants or likes.
It'll be thoughtful on your part, your confusion gets solved & he gets something that he wants.

If you are in the UK, then thats a no brainer. Get him some toothpaste and floss and a "how to guide" for the two.
I have the same problem...Last year I got my fiance a D&G watch, this year budget is not so big but was thinking of getting him cufflinks engraved...?
